VEHICLE DESCRIPTION
On completion of this training and assessment you will be able to operate a powered industrial forklift truck:

Eligibility for a high risk work licence
You are eligible to obtain a licence if you:
- are at least 18 years of age
- can use English at a level that enables the safe performance of high risk work
- have the necessary knowledge and ability to safely perform the high risk work
- have been assessed as competent by an assessor working for Len’s Driving School using the relevant assessment instrument under realistic workplace conditions
For any High Risk Course you must complete an evidence of identity (EOI) form and present original documents showing your name, photo, date of birth, signature and current address to the value of 100 points to the trainer or assessor.
You must provide original documents, not photocopies or certified copies.
Commonly used combinations of evidence of identity documents include:
- an Australian birth certificate and an Australian driver’s licence, or
- an Australian drivers licence, Medicare card, ATM card and a utilities bill.
The table below outlines the acceptable evidence of identity (EOI) documents and points value allocated to each document. The combination of EOI documents must contain your date of birth, current residential address, photograph and signature.
The documents you provide generally should be the same documents you provide to the Australia Post customer service officer when lodging your Application for National Licence to Perform High Risk Work – New Application form.
If you use more than one credit card, savings account card or statement, they must be from different financial institutions.
- If you use more than one utilities bill, they must be from different utilities providers.
- ALL documents provided must contain your name in full, not just your initials. The only exception may be a Council rates notice.

TRAINING AND ASSESSMENT
This licence will allow you to operate a forklift truck safely including driving and manoeuvring, picking up and placing of loads at various stack heights. You will learn:
- Identification of workplace hazards and risk management
- Machine types and licence classes
- Forklift stability
- Forklift data plate, checklists and pre checks
- Workplace hazard identification and forklift truck pre-start checks
- Mounting/dismounting the machine
- Operating controls
- Travelling in machine
- Danger points and positioning when steering
- Basic positioning of machine, shut down and secure forklift truck procedures
- Shift loads
- Correct post-operational, shutdown an site security procedures
- Operation, Shut down, Secure
You’ll be trained by a qualified trainer and assessed by a NSW WorkCover accredited Assessor. This Assessor is contracted to Len’s Driving School and their fees are included in your course fees.
